Registration and Payments
Registration for educational activities may be made by mail, telephone, fax, or in person. Payment must be made prior to the day of the course via check, money order, credit card or training voucher authorization made payable to: Treasurer, Howard University and mailed to the Office of Continuing Medical Education at the address above. Only Visa and MasterCard are accepted.

Requests for refunds must be made prior to the first day of the activity. Cancellations prior to the first day are subject to 10% administrative fee or may be applied to future activities. Cancellations after the activity begins are subject to forfeiture of the entire registration fee. Confirmation of registration will be made up to one week prior to the start of the activity. After that time, please call for confirmation.

HUCME reserves the right to cancel activities due to inclement weather or other reasons. Full refunds will be made if an activity is cancelled by HUCME.

Credits for Physicians
Credit for attendance at sessions should be reported by the individual registrant to the requesting organization(s). A signed certificate of participation is provided by HUCME to physicians who complete the Record of Attendance distributed at the educational activity.

Disclosure
All speakers and planning committee members participating in CME activities sponsored by the Office of Continuing Medical Education, Howard University College of Medicine, are expected to disclose to the audience any real or apparent conflict(s) of interest related to the content of their presentation(s). Full disclosure of their relationships will be made before or during the activity as needed. Speakers expecting to discuss “off-label” drug usage must identify that portion of the presentation as being related to “off-label” drug usage.
