DC-GAPS is a family-centered medical home that is a “one-stop shopping” approach to providing comprehensive primary care. The medical home team forms a partnership with a child and his or her family to assure that all of the medical and non-medical needs of the patient are met. The medical home team includes primary care physicians, specialists, sub-specialists, hospitals, healthcare facilities, public health professionals and the community.

About Our Medical Home Providers
Primary Care Doctor
A patient’s first contact for health concerns, as well as care for other various medical conditions, both diagnosed and undiagnosed.
Specializes in the treatment of blood diseases and diseases of certain organs that form the blood such as the spleen and bone marrow.
A nurse who can treat both physical and mental conditions by evaluating medical history, conducting physical exams, ordering tests, and providing a diagnosis, treatment, and recommendations for diseases, and can also prescribe medications.
Assists with access to health care and makes referrals to government, educational, and social agencies.
Provide education to the patient and family explaining the inheritance and diagnosis of the disorder. This may include taking a family history and providing support for the family and patient.
